Importance of GP34s

When submitting your bundle of prescriptions to P&CFS, one of the things that should always be included is your yellow GP34 form, as it is a legal requirement. You will need to have counted your prescription submission before completing, which involves totalling your number of forms and items.
However, there are some forms which are not submitted, or aren’t counted:
Do not submit or count any UCF forms, these are paid electronically.
Any MCR serial prescriptions should be counted as 1 form, but 0 items. MCR serial prescription items are paid electronically, however the forms should be submitted once the instalments are completed.
Any PC70 forms from CD instalment prescriptions should be submitted but not counted. Make sure to remove any staples, paperclips or stationery attaching the PC70 to the prescription form.
You do not need to add in instalments to your total form or item count. These will be picked up by P&CFS through your endorsements and paid from these.
Count any chargeable prescriptions, such as FP10s separately, as there is an additional space to list these on the GP34.
Count your GP10A Stock Orders separately, as there is an additional space to list these on the GP34.
Once you have counted and prepared your bundle, on your GP34 you should include:
Your contractor code.
Your pharmacy name and address, including post code
The month you are submitting the bundle for, and which submission it is.
Your pharmacy stamp
The total number of forms included in your bundle, in the appropriate fields.
The total number of items included in your bundle, in the appropriate fields.
If you run out of GP34 forms, these can be ordered from your Health Board stores department, contact details can be found here. If you are unable to source a form in time for your prescription collection, you can borrow one from a neighbouring pharmacy. If this is not possible, you can also write the details needed on a blank piece of paper, with a note to say this was used due to being unable to procure a form.
